Growing together.Position Highlights:Practice with a multi-disciplinary team of heath care providers that includes Primary Care Physicians, RN Case Managers, social workers, and clinic staffOur model is designed to provide the most efficient, comprehensive, and proven care techniques to treat the whole patient - physically, mentally, and socially - at each visitPrimarily manage care in the patient’s home; however, care may require an office or other facility setting depending on the needs of the patientProvide care for some of our sickest patients - patients whose needs are beyond what our care management and clinic-based resources can provide, but whom hospice services are not appropriateDiagnose and treats both acute and chronic conditions such as diabetes, hypertension, kidney disease and other chronic medical conditionsTreat acute conditions such as minor injuries and other acute medical conditions such as infections or exacerbations of chronic diseasesFacilitates patient and family decision making regarding treatment decisions, end of life care, and serious illness discussionsPrescribes pharmacological and non-pharmacological interventions
