At Optum, we are transforming healthcare nationally while providing physician-led care locally.Primary Responsibilities:Provide care that is compassionate, appropriate, and effective for the treatment of pulmonary/ critical care related health problemsCompliance with all coding and documentation requirementsPractice evidence-based medicineIn and outpatient mixPractices in accordance with the respective state laws and regulations governing the practice of advanced practice nursingEstablishes and maintains effective relationships with members and their legal representatives, facility staff, PCPs, and other collaborating physicians/providersCompletes follow-up and post-hospital assessments according to documented standard operating procedureProvides counsel, support, and education to facility staffSupports FCMs in developing and providing in-service educational programs to facility staffParticipates in management meetings that include, but may not be limited to:
